La especialidad de Prótesis Estomatológica en Sancti Spíritus: apuntes para su historia

RESUMEN Prótesis dental es la rama de la Estomatología que se encarga del estudio de los principios fundamentales de la rehabilitación de los pacientes con ausencias dentarias, y de la prevención, diagnóstico y tratamiento de las alteraciones del sistema estomatognático. Se realizó una investigación cualitativa con el uso de métodos teóricos -histórico-lógico, analítico-sintético- y empíricos -revisión documental y entrevistas a directivos, académicos y asistenciales, residentes y egresados. Para su mejor entendimiento, se dividió el desarrollo histórico en tres etapas: la primera describe la creación de instalaciones de salud, la fundación de la docencia de pregrado y posgrado, así como la formación del grupo provincial y el capítulo espirituano de Prótesis Estomatológica, necesarios para estimular el desarrollo científico-técnico de la especialidad. La segunda, aborda el incremento de servicios y la incorporación de nuevas tecnologías para la formación integral de los especialistas; y la tercera, la consolidación del proceso de formación de pregrado y posgrado, que garantiza la atención de los pacientes con necesidades de tratamientos rehabilitadores en la provincia. La especialidad de Prótesis Estomatológica ha tenido un desarrollo ascendente en la provincia de Sancti Spíritus, y cuenta con un claustro docente fortalecido para la impartición de la docencia de pre y posgrado. Por otra parte, se ha mantenido una formación estable de los recursos humanos, en correspondencia con las necesidades de atención a la población, mostrando resultados científico-técnicos que avalan el trabajo actual de la especialidad.

ABSTRACT Dental prosthesis is the branch of Stomatology that is responsible for the study of the main principles of the rehabilitation of patients with dental absences, as well as for the prevention, diagnosis and treatment of alterations of the stomatognathic system. Qualitative research was carried out using theoretical methods-historical-logical, analytic-synthetic and empirical-, documental review, interviews with managing, academic and health care staff, residents and graduated. For its better understanding, the historical development was divided into three stages. The first stage describes the creation of health facilities, the beginning of undergraduate and postgraduate teaching, and also the creation of the provincial group and chapter of Dental Prosthesis of Sancti Spiritus, necessary to stimulate the scientific-technical development of the specialty. The second approaches the increase of services and the incorporation of new technologies for the comprehensive treatment of the specialists; and the third one the consolidation of the process of undergraduate and postgraduate training, which safeguards the care of patients with needs of rehabilitation treatments in the province. The specialty of Dental Prosthesis in the province has had an upward development in the province of Sancti Spiritus, and has a teaching staff strengthened for imparting undergraduate and postgraduate teaching. On the other hand, a steady training of the human resources has been kept, in correspondence with the needs of care of the population, and showing scientific-technical results that support the current work of the specialty.

Integración de tres especialidades estomatológicas en el tratamiento protésico / Integration of three stomatological specialties in the prosthetic treatment

Los implantes dentales son una opción de restauración estética, funcional y armónica, cuya tasa de éxito es superior a 95 %. Se presenta el caso clínico de una paciente de 24 años de edad, atendida en la consulta de Ortodoncia de la Clínica Estomatológica Provincial Docente Mártires del Moncada de Santiago de Cuba, quien fue remitida desde el Servicio de Prótesis Estomatológica, con el fin de lograr el espacio adecuado para colocarle un implante, pues desde los 12 años de edad aproximadamente había perdido un diente a causa de caries. La restauración protésica mediante el implante dental proporcionó un excelente resultado.

Dental implants are an option of cosmetic, functional and harmonic repairs whose rate of success is higher than 95 %. The case report of a 24 years patient is presented. She was assisted in the Orthodontics Service of Mártires del Moncada Teaching Provincial Stomatological Clinic in Santiago de Cuba and was referred from the Stomatological Prosthesis Service, with the purpose of achieving the appropriate space to place an implant, because she had lost a tooth due to dental decay since she was approximately 12 years. The prosthetic repair by means of dental implant provided an excellent result.

Software educativo sobre instrumental y materiales para prótesis estomatológica / Educational Software about dental prostheses materials and instruments

RESUMEN Introducción: la producción del software educativo facilita al alumno la asimilación de los conceptos, consultas de documentación auxiliar y realización de ejercicios. Objetivo: elaborar un software educativo del tema instrumental y materiales para prótesis estomatológica de la carrera de Estomatología. Método: se realizó una investigación de desarrollo tecnológico en el campo de recursos del aprendizaje en la Clínica Estomatológica "Artemio Mastrapa Rodríguez", en Holguín, Cuba, desde enero de 2016 a enero de 2017. La muestra fue de 28 estudiantes y 18 profesores. La selección fue intencionada y aleatoria simple. Se utilizaron métodos teóricos como: revisión bibliográfica-documental, inducción-deducción, análisis-síntesis y el cuestionario como método empírico. Resultados: se desarrollaron los contenidos de acuerdo con los objetivos esenciales y métodos de enseñanza y se conformaron los módulos del software. El 92,9% de los estudiantes y el 88,8% de los profesores calificaron el recurso de excelente. Conclusiones: se elaboró un software educativo que puede ser utilizado durante el desarrollo del proceso de enseñanza-aprendizaje y el estudio independiente. La mayoría de los profesores y estudiantes evaluaron el software educativo de excelente.

ABSTRACT Introduction: the educational softwares facilitate comprehension. Provide students a helpful tool for individual study and exercises solving. Objective: to create an educational software about dental prostheses materials and instruments for dentistry career. Method: research was carried out in the field of learning resources, at "Artemio Mastrapa" dental clinic in Holguin, from January 2016 to January 2017. Sample was 28 students and 18 teachers, all selected according to the purpose of the study. Theoretical methods were used, such as bibliographical and documental review, analysis and synthesis, induction and deduction and individual interviews as the empiric method. Results: creation of the software module with contents according to the signature objectives and the teaching methods. The 92.9% of students and 88.8% of teachers considered it excellent. Conclusions: the creation of an educational software for students and teachers. It was considered excellent by most of students and teachers interviewed.

Satisfacción de usuarios y proveedores de un servicio estomatológico de prótesis / Users and suppliers satisfaction in a stomatological prosthesis service

Se realizó un estudio descriptivo y transversal que incluyó a 2 especialistas en prótesis y 2 licenciadas en atención estomatológica, pertenecientes al Departamento Clínico de Prótesis del Servicio Estomatológico del Policlínico Docente Julián Grimau García de Santiago de Cuba, así como de 321 pacientes con atención concluida en dicho Servicio, para evaluar la satisfacción de proveedores y usuarios durante el 2015 en relación con la atención brindada y recibida, respectivamente, para lo cual se establecieron criterios, indicadores y estándares. Tanto los profesionales como los usuarios quedaron satisfechos, ambos con 5 de los criterios evaluados de adecuados, ya que superaron el estándar establecido (100 y 90 por ciento, en ese orden).

A descriptive and cross-sectional study that included 2 specialists in prosthesis and 2 graduates in stomatological care, belonging to the Prosthesis Clinical Department of the Stomatological Service in Julián Grimau García Teaching Polyclinic was carried out in Santiago de Cuba, as well as of 321 patients with care concluded in this Service, to evaluate the satisfaction of suppliers and users during the 2015 related to the care offered and received, respectively, for which criteria, indicators and standards were established. Either professionals or users were satisfied, both with 5 of the evaluated criteria as appropriate, since overcame the established standard (100 and 90 percent, in that order).

Sobredentadura: alternativa terapéutica para disminuir las extracciones dentarias / Overdenture: therapeutic alternative to decrease tooth extractions

Se describe el caso clínico de una paciente de 45 años de edad, atendida en la Clínica Estomatológica Docente "José Luis Tassende" de Santiago de Cuba porque refería que en innumerables ocasiones se le fracturaban los dientes artificiales de la prótesis y la porción coronaria de la espiga del incisivo central superior derecho; también presentaba algunos dientes remanentes con facetas de desgastes en las caras palatinas, incisales y vestibulares, por lo cual se le realizó una prótesis parcial metálica (superior e inferior), con sobredentadura sobre dientes naturales. Con el proceder aplicado se evitaron las extracciones dentarias y se logró una adecuada rehabilitación estética y funcional.

The case report of a 45 year-old patient, assisted in "José Luis Tassende" Stomatological Clinic from Santiago de Cuba is described. She complained of fractures in countless occasions of the artificial teeth from her denture and the coronary portion of the pin of the central right superior incisor tooth; she also presented some residual teeth with facets of waste in the palatine, incisal and vestibular faces, reason why a metallic partial prosthesis (superior and inferior), with overdenture on her natural teeth was made. With this procedure, the teeth extractions were avoided and an appropriate aesthetic and functional rehabilitation was achieved.

Evolución de pacientes con sobredentadura e implantología con carga inmediata / Progress of patients with overdenture and immediate load implant

Se realizó una investigación descriptiva, longitudinal y prospectiva en la Clínica Estomatológica Provincial Docente "Mártires del Moncada" de Santiago de Cuba en el período comprendido desde septiembre de 2010 hasta julio de 2012, a fin de evaluar la evolución de 17 pacientes tratados con implantes roscados marca MIS, con abutmen o atache de bola, cargados inmediatamente, para lo cual se convirtió la misma prótesis que portaban en una sobredentadura mandibular soportada en la mucosa y retenida por el implante, pues su aparato protésico convencional carecía de la funcionalidad deseada. La rehabilitación con ese tipo de sobredentadura fue efectiva en aquellos pacientes con problemas en los principios biomecánicos de retención y estabilidad, así como también en los que presentaban una buena disponibilidad de hueso y en los que se colocaron implantes de mayor longitud, todos los cuales respondieron mejor al tratamiento; por consiguiente, se elevó su calidad de vida al proporcionarles confort y seguridad con el uso de su aparato protésico.

A descriptive longitudinal and prospective study was conducted in "Martires del Moncada" Provincial School of Dentistry of Santiago de Cuba from September 2010 to July 2012 in order to evaluate the progress of 17 patients treated with threaded implants MIS, abutmen or ball attachment, immediately loaded, for which the same prosthesis became a mandibular overdenture supported on the mucosa and retained by the implant, because their conventional prosthetic device lacked the desired functionality. The rehabilitation with this type of overdenture was effective in those patients with compromised biomechanical principles of retention and stability, as well as in those having a good bone availability and in those with greater length implants, thus responding better to treatment. Therefore, their quality of life increased by providing comfort and safety with the use of this prosthetic device.

Evolución de pacientes tratados con implantes unitarios de carga inmediata en la Clínica Estomatológica Provincial de Santiago de Cuba / Progress of patients treated with immediately loaded single implants in the Provincial Stomatological Clinic from Santiago de Cuba

Se realizó un estudio descriptivo, longitudinal y prospectivo para describir la evolución de 55 pacientes tratados con 65 implantes unitarios de carga inmediata (marca MIS o Nano), colocados sobre huesos cicatrizados en la Clínica Estomatológica Provincial Docente de Santiago de Cuba durante el período comprendido desde septiembre de 2010 hasta enero de 2012. Entre los criterios más desfavorables para considerar la estética inadecuada en algunas restauraciones, sobresalieron el color y la posición de estas; asimismo, la higiene bucal deficiente, la masticación de alimentos duros y la no asistencia a los controles clínicos fueron las indicaciones más incumplidas. La rehabilitación con implante unitario de carga inmediata resultó exitosa en 86,2 % de los integrantes de la casuística y 38 de ellos (74,5 %) se mostraron satisfechos con los resultados terapéuticos de la técnica, cuya aplicación contribuyó a la integridad biopsicosocial y calidad de vida de los pacientes.

A descriptive, longitudinal and prospective study was carried out to describe the progress of 55 patients treated with 65 immediately loaded single implants (MI or Nano types), placed on healed bones in the Teaching Provincial Stomatological Clinic in Santiago de Cuba during the period from September, 2010 to January, 2012. Among the most unfavorable approaches to consider the inadequate aesthetics in some restorations, there were the color and their positions; also, the scarce buccal hygiene, the chewing of hard foods and the non attendance to the clinical appointments were the most unfulfilled indications. The rehabilitation with immediately loaded single implant was successful in 86.2% of the members of the case material and 38 of them (74.5%) were satisfied with the therapeutic results of the technique which application contributed to the bio psychosocial integrity and life quality of the patients.

Cambios suscitados en la formación del capital humano que confecciona prótesis estomatológica en Villa Clara / The changes in the formation of human resources for prosthodontics in Villa Clara

La formación del capital humano dedicado a la confección de las prótesis estomatológicas con una preparación política-ideológica, ética y científica según los principios de sociedad cubana, incide en la eficacia de los graduados y en el aumento de la calidad de vida de la población. Este trabajo realizado por docentes de la Facultad de Tecnología de la Salud "Julio Trigo López" consistió en una investigación descriptiva con enfoque histórico lógico y resume las etapas fundamentales por las que transitó este proceso en Villa Clara desde 1977 hasta 2010. Los datos fueron obtenidos a partir de un análisis documental de los hechos históricos, se consultó el registro nominal de graduados, los planes de estudio y la fundamentación para la carrera de Técnicos Medios y Licenciados en Prótesis Estomatológica. El procesamiento de la información analizada permitió apreciar que esta especialidad ha tenido un creciente desarrollo durante el período estudiado.

The formation of prosthodontists with a political-ideological, ethical and scientific preparation according to the principles of the revolution have a great incidence in the efficiency of this personnel as well as in the elevation of life quality in the population. This work was carried out by the professors of "Julio Trigo López" Health Technology Faculty. This is a descriptive research work with a historical-logical approach. It summarizes the different stages of the research work in Villa Clara in the period from 1977 to 2010. The data was collected through the analysis of documents of the historical events, there were consulted the records of graduated students, the study plans and the foundation of the prosthodontics career for technicians and bachelors. The analysis of the information showed that this specialty has had a steady development during this period.
